What the Nordic Method Is

The Nordic Method is regenerative: it acts on the tissue itself instead of producing an erection each time with ED medication, a pump, or an injection. It uses stem cells taken from the patient's own fat and acts on the nerves and blood vessels involved in erections, so the body can do more of the work itself. Because it is a medical procedure, it begins with a full evaluation and continues with follow-up afterward, which includes consultations at 3, 6, and 9 months.

The Danish Phase 1 Trial

The trial was led by Dr. Søren Paludan Sheikh and colleagues at Odense University Hospital in Denmark, and it is known as the Haahr et al. trial. It was approved by the Danish Medicines Agency, partly funded by the Danish Cancer Society, and registered on ClinicalTrials.gov under NCT02240823.

The trial followed 17 men who developed erectile dysfunction after a radical prostatectomy, the surgery that removes the prostate. Their ED was severe, with a median score of 7 on the IIEF-5, a short five-question test that doctors use to rate how serious ED is. All of them had already tried conventional treatments such as ED medication (Viagra, for example) without getting enough benefit. Each participant then received a single injection of about 22 million stem cells taken from their own fat. The results were published in EBioMedicine in 2016, followed by a 12-month report in Urology in 2018, and a 2017 expert review in Sexual Medicine Reviews discussed the approach as an option for men whose ED doesn't respond to ED medication.

What the Results Show

Within six months, 73% of the continent patients, meaning those who had regained bladder control after surgery, recovered erectile function good enough for intercourse. Their median IIEF-5 score rose from 7 to 17, which falls in the functional range, and at 12 months more than half of the continent patients could still have intercourse.

The participants had severe ED after prostate surgery, which is one of the hardest situations to treat, and that makes the outcome encouraging.

How Does the Nordic Method Work?

With the research in view, the next question is what treatment actually looks like, from the first consultation to the follow-up afterward.

Step 1: Consultation and Medical Evaluation

The first step is a conversation. A doctor reviews your medical history and checks whether the treatment suits you, and your erectile function is scored with the IIEF-5 so there is a clear starting point to measure against. Depending on your age and health, the evaluation may also include heart screening and blood tests.

Step 2: Liposuction and Stem Cell Isolation

On the treatment day, a surgeon removes about 400 g of fat from the abdomen. This happens under general anesthesia at CIMA Hospital, so the patient is asleep and monitored by a hospital team throughout. Fat is used because it holds plenty of stem cells, and those cells are separated in our laboratory the same day, so nothing is stored or banked.

Step 3: Injection Into the Erectile Tissue

Once the cells are ready, they are injected directly into the erectile tissue under local anesthesia, a step that takes about 30 to 45 minutes. Placing them right where the damage is means the cells don't have to find their own way there.

Step 4: Monitoring and Follow-Up

Repair takes time, so we keep track of progress after treatment. Men take home a device, included in the treatment price, that records nighttime erections, which are among the earliest signs that nerves are working again. Follow-up consultations at 3, 6, and 9 months compare that data with the IIEF-5 score, so progress is measured and not left to guesswork, and most men can expect results to build over four to six months.

What Is the Science Behind the Nordic Method?

Erections depend on nerves that send the signal and blood vessels that carry the blood, and surgery, diabetes, and aging can damage both, so the Nordic Method acts on both.

Angiogenesis

Angiogenesis is the growth of new blood vessels. When the vessels supplying the penis are narrowed or damaged, the tissue can't fill with blood the way it should. Stem cells release growth factors and other signals that act on new vessel formation, and a better blood supply gives the tissue a stronger chance of responding again.

Neuroregeneration

Neuroregeneration is the repair of damaged nerves. A nerve that has been stretched or injured may send a weak signal or none at all, and no pill can make up for a signal that never arrives. The stem cells act on the nerve's recovery, and because that is a slow biological process, improvement builds over four to six months.

Who Is the Nordic Method For?

The Nordic Method was developed for one group of men and may be relevant to others, though the evidence differs for each. Here is where things stand for the three most common causes of ED.

Men With ED After Prostate Surgery

This is the group the treatment was developed for, and it is the only one with published trial data. Prostate surgery takes place right next to the nerves and blood vessels that control erections, so even careful surgery can damage them. Many men are told to give it time, and some do recover, but many don't, and Viagra (sildenafil) and Cialis (tadalafil) help only about half of men after a radical prostatectomy. The Nordic Method was developed for men whose ED came from that kind of physical injury. The sooner you act, the better, and there's no reason to wait. We assess timing individually, based on your surgical history and recovery. In the trial, results were reported for men who had regained bladder control, and outcomes are less favorable with incontinence.

Men With Diabetes-Related ED

Diabetes slowly damages small blood vessels and nerves, and erections depend on both, which is why ED medication often stops working as the disease progresses. The Nordic Method acts on the same nerves and vessels, so the mechanism fits some men with diabetes-related ED, depending on how much damage there is and on their general health. There is no clinical trial data for diabetic ED yet. For that reason, we evaluate each case individually.

Men With Age-Related ED

For many men, ED comes from gradual changes in the blood vessels or from long-term illness, and aging alone is rarely the whole explanation. The Nordic Method acts on the same nerves and vessels, so the mechanism fits, but no trial has measured results for age-related ED yet, and dedicated trials are planned. There is no strict age limit for treatment, because overall health and heart health matter far more than the number of years.

Why Is the Treatment Offered in Costa Rica

In the United States, most regenerative cell treatments for ED are limited to research settings, and the American Urological Association's 2018 guideline classifies stem cell therapy for ED as investigational. What Are the Risks and Side Effects of the Nordic Method?

The Danish trial reported no serious adverse events related to the cell treatment, but the procedure still has two physical steps, and each one can cause minor effects.

Liposuction Site Effects

Soreness, bruising, and swelling where the fat was taken are common afterward, and they usually settle within days to weeks. Infection and bleeding are uncommon but can happen with any liposuction, and general anesthesia carries small risks of its own, which our team reviews with each patient beforehand. That is why this step takes place in a hospital.

Injection Site Effects

The injection itself is brief and usually well tolerated. The most common reaction is temporary redness or slight swelling at the injection site.

Who May Be Advised Against Treatment?

Not everyone is a candidate. We may advise against treatment for men with active cancer, certain serious heart conditions, bleeding disorders, or infections that raise surgical risk, and those whose ED is mainly psychological or caused by a medication may not benefit either. Penile fibrosis, incontinence, or no erectile function for more than three years also lower the chances of success. Hearing that treatment isn't a good idea can be disappointing, but we consider it sometimes the safest and most responsible outcome.
